Product description...

A Case Management Guide to Better Manage End of Life Care in America

End of life care has made vast improvements; however, statistics show that aggressive, high-cost care for patients continues to be widespread. We continue to see very ill patients in and out of the hospital or rehab units for their final weeks to months of life. Proactive discussions with patients and families can identify wishes and goals earlier and provide them with more control, comfort and dignity.

Case Management, A Palliative Perspective serves as a resource for case managers or anyone involved with individuals facing a life threatening or life limiting disease. This book offers approaches and options for these patients and families and the description of the insurance maze helps anyone addressing end of life planning needs. In a fast-paced, complex medical culture, it can help the case manager shift the focus on end of life care for patients and families.

“Having worked side by side with Rhoda Neader, I can say without a doubt that this book reflects all the knowledge, experience, and humanity she brought to her work helping the most vulnerable of patients. Using case histories from her wide-ranging career, she illustrates just how crucial the case manager is for the palliative care team and offers concrete examples of means for resolving the most complex issues the team may face. Palliative Care professionals across all disciplines, whether novice or seasoned, will find her wisdom enhancing their own practices.”
– Ned Stolzberg M.D., Executive Medical Director, Hospice of the Valley, Phoenix, Arizona

“Rhoda is a smart and sensitive person, and this comes through in her book, Case Management: A Palliative Perspective. The book is a practical guide to case management in palliative care, a demanding and rewarding area of practice. It is a useful “how to” in organizing and managing patients and their families at this important time in their lives.”
– Pat Winstead-Fry, RN, PHD, former Dean of Nursing, University of Vermont

“Rhoda puts life in this book with her wealth of knowledge and hands-on experience, striking a great balance of case management and palliative care. Sharply written, a must read for all care providers especially in the hospital setting.”
– Ajitpal Mann M.D., Medical Director, Critical Care, Abrazo Arrowhead Hospital, Glendale, Arizona
